Neoplasms coding is complicated; and the rules and guidelines for applying the codes are complex. As payer’s scrutiny increases and automation becomes more advanced in 2026 pushing for productivity, paying close attention to details and specificity in documentation will be required, otherwise medical errors will not only lead to increased denials, financial loss and compliance risks but it could also compromise the quality of care.

Breast cancer documentation is like a puzzle and unless detailed documentation with the highest-level of specificity for example, describing the quadrants of the breast in clock positions is documented, medical errors will increase.
