Valve Replacement with Radiofrequency ablation of Pulmonary Vein Isolation

Dr performed a Mitral Valve replacement (CPT code 33430) and Tricuspid Valve replacement (33465) both were on cardiopulmonary bypass. A radiofrequency ablation, pulmonary vein isolation (33257) was also done, I believe these CPT codes to be correct. The patient was also cannulated the report reads " Pt was cannulated with aortic cannula for IVC and for SVC, and ascending root cardioplegia catheter. Retrograde cardioplegia catheter was placed via the right atrium into the coronary sinus - I have struggled with the CPT code and have used 33954 - Does someone know if these codes are correct Thanks
