Dx: Ulcerating mass of the right thumbnail

Procedure performed:
Excision of the ulcerating mass, revision of the nail bed defect, and ablation of the germinal matrix of the nail bed

Detail of procedure:
the patient has a nail defect with ulcerating lesion. This was then irrigated out, cleansed, and the lesion was then excised in a wedge fashion. this was then sent to Pathology for analysis. The nail bed was identified. this was excised and also ablated with bipolar cautery. Once this was accomplished, the nail bed was then rotational flap-wise inset to close the defect by using 4-0 prolene sutures. Upon completion of this, the skin flap that was created over the germinal matrix was then closed with 4-0 prolene sutures also. Upon completion of this, the wound was then dressed sterilely. the tourniquet was let down. A thumb spica gauze splint was then applied.
