Medicare allows for transitional care management, 99495 or 99496 to be billed during the same time as monthly physician dialysis billing, 90960-90962. Is the TCM face-to-face visit able to be preformed at the dialysis unit (POS 65)? Or does the patient need to come into the office (POS11)? I cannot find any documentation regarding this. Any help is appreciated!
