Patient comes into the office for transitional care management following a hospital discharge. Provider codes E11.65 (Type 2 DM with hyperglycemia) and E11.628 (Type 2 DM with ketoacidosis without coma). Is it more appropriate to code the reason for the hospital admit/follow up (the ketoacidosis), or the state of the patient at the TCM visit (DM w/hyperglycemia)?
