Any suggestions? I am leaning towards 27310 because the components was explored, and cleaned. Extensive synovectomy was also performed along with Poly liner exchanged.
 
I would need to see the Operative Report to be certain, but most likely what you are seeing is an Arthrotomy with Extensive Synovectomy (27335). In the presence of a TKR, in order to do an "extensive synovectomy including the posterior compartments," the Tibial Polyethylene Component needs to be removed "temporarily" until the posterior compartments have been explored and treated. A new Tibial Polyethylene Component is usually placed when this has been done. Reusing the previous one is a "No No," as it may have some wear, and it can not be re-sterilized for re-insertion without risk of damage from the Autoclave process. This does not constitute a Revision Arthroplasty.
