Good morning,
I was wondering if I could get a clarification on when to use 26480/25310 with 25447? Does it matter which tendon or where the incision was done?

From personal experience, I use 25310 if the tendon is transferred from the forearm/wrist, and 26480 if at dorsum of hand. The type of tendon, whether flexor or extensor, does not have any influence in this situation.
 
We use 26480. I've been told that tendon transfers with hands depend on "where" the tendon is being transferred to rather than "from" where it is transferred.
