I cannot find published yet, online anyway on any NDC crosswalks what CPT vaccine code i should be using for this vaccine for 2021-2022. I believe it will be 90756 or 90674. When I looked at the company Siqirus list the NDC numbers they have tied to the 90756 and 90674 are not the NDC number of my product. Can anyone help with this?
 
I ususally don't map to NCD, but report the vaccine formula as described in CPT. The difference between 90756 and 90674 is that 90674 is antibiotic and preservative free, where as 90756 is just antiboitic free. Do you have the vials (and box) yet? Generally that's how you can tell.

I knew the difference because of the verbiage in CPT book. I thought we were suppose to make sure the NDC numbers mapped . Good to know. thank you. I will take a look at the box and not worry about linking NDC.
