Hello,

Have you all had any luck getting paid from BCBS and UHC for telemedicine services? If yes, can you tell me if you are billing E&M codes with the 95 modifier, or are you billing 99421, 99422 and 99423 / 98970, 98971 and 98972?

CPT codes 99421, 99422, 99423 are for Digital E/M services. This is for patient portal or e-mail inquiries, initiated by the patient. You can only bill once per 7 days and it is cumulative of time spent in digital communication with the patient during the 7 days. Telemedicine services (synchronous/live audio/video) is billed with E&M codes (i.e., 99213-95) with a 95 modifier. We are just initiating telehealth in our practice and have not yet billed any charges so I do not know yet regarding reimbursement.
 
I recently read that we should use modifier GQ for telemedicine and the GT was for institutional claims.... I have not seen much on modifier 95 any details on that would be helpful.
