I am stuck on codes for this op note. Leaning towards 26140 but i dont like it because he didnt do any reconstruction.
Maybe 26140-52?

incision made over Ip joint on ulnar aspect.

dorsal capsule opened longitudinally and joint explored. Purulence found in joint. Irrigated. synovium irritated and resected. palmar capsult intact. volar plate intact. collateral ligaments intact. EPL intact. Infection from joint didnt penetrate into the osseous strictures proximally or ditally nor into the nail forming tissue.
a complete synovectomy had been obtianed. Wound left open with a pen rose drain.
 
Sounds like he's in the IP joint, so possibly 26080 Arthrotomy with exploration, drainage or loose body removal. This code includes synovectomy under AAOS guidelines. If you think it's appropriate, you should review it with your doctor to see if he's in agreement.
