STSG is applied to wound that is 188.5 sq. cm. Would you bill 15100 and 15101 or only the 15100? Do you need a full (additional) 100 sq. cm. in order to bill the 15101?

15100 & 15101 as supported by the following which is documented in Principles of CPT Coding (AMA)

...15101 & 15121 are add on codes used in conjunction with 15100 & 15120 for split grafts that measure beyond the initial 100 cm2 up to and including an additional 100 cm2.
