My physician was doing a PLIF the disc space was prepared with the interspace shapers. The implant was inserted after the trial, it was expanded but it felt loose therefore it was removed. The implant was not defective as it appeared to be expanded properly on inspection. However, this disc space apparently was too concave to grab the endplates. Therefore the it was packed with bone graft only. My question is do I charge for the implantation of the implant? Implant and removal? or nothing at all.?

This is difficult without the note. If I am reading and interpreting this correctly. The provider performed a PLIF with attempted insertion of the cage so I would code 22853-53.
