If the independent lab has billed for sample collection G2023 and its been denied as inclusive with test codes, then this denied claim cannot be rebilled under physician office with 99211. Please confirm if my understanding is correct.

Who collected the specimen?

Did the patient have an office visit on the DOS in question?
