Specific to refractive lens coding: Could someone please share an example of an upgrade within a HCPCS code such as V2750? Would there be a total of 4 units? Would you GK the base V2750 with the official HCPCS description and add the additional description to the upgrades V2750 when billing? Assume for this example there is a good ABN for each V2750. Physician ordered V2750 but there is no medical necessity.

V2750 GA RT (upgrade to polar backside noglare)
V2750 GK RT (anti reflective coating per lens)
V2750 GA RT (upgrade to polar backside noglare)
V2750 GK LT (anti reflective coating per lens)
