Wiki sleep study is done first and then evaluation

Hello,
I do the coding for sleep medicine and often the clinic does the sleep study first 95811 and then a couple days later does the evaluation 99204. But then the 99204 is denied by the insurance as service is not new but establish. My question is if the sleep study is done first should I bill the evaluation as a established visit?
 
They should have a visit first to determine if a sleep study is needed. That is your new patient visit. Then the sleep study is done, then there is a visit to go over the results.
