Hi I have a questions for Skilled Nursing Facility code E/M. Doctor in the first encounter transfer a patient to another institution "hospital" a same day. Describe encounter "Transfer Note"? How you a coding CPT code to E/M ? Thanks for your help?

I do auditing for skilled nursing. In my experience, when our patients are transfered to the hospital there is a full visit completed, usually coded 99310.
