Wiki sinus cranialization in the setting of a trauma

Hello, does anyone know what are the best option to code for a Sinus Cranialization in the setting of a trauma - the surgical approach will be a skull base approach, best reflected in CPT code 61582. However, billing form the skull base surgery section of the CPT code book requires both the approach code and a definitive procedure code and the repair work does not have a definitive procedure code. The repair work could be partially encompassed in the following codes for fracture treatment, obliteration of sinus, and cranioplasty, but this neglects the extensive amount of work involved in the opening and closure; and a 22 modifier seems inadequate given that this will be an all-day surgery.
