Good morning everyone,

I am in the middle of a debate with our prosthetics department and any opinions will be greatly appreciated.
When our podiatrists or orthotists issue off the shelf orthotic shoe inserts [L3040} are they issued / billed per pair? or per insert? I am including the code and description per Optum 360 Encoder Pro code book.

L3040 Foot, arch support, removable, premolded, longitudinal, each

I argue they are coded per insert because the code says "each" and is referring to singular "arch support".
The orthotist argues they are issued per pair because they are off the shelf.
