I am getting a denial for CPT 36247,50 with ICD10: D25.9 and N92.0 from Aetna( Medicare Advantage PPO) not being medically necessary- to check correct coding LCD# L35092. Has anyone had this issue?
Any resources or feedback will be greatly appreciated.

Google LCD L35092. It will contain the billing guidelines for this CPT code and will have a list of what diagnoses they consider to be "medically necessary" for this procedure
