My doctor coded a scar as keloid (701.4) and wants to code the excision as 28045 (excision of tumor). From the research I have done, it seems that 11400-11406 (excision of benign lesion) is the correct code for this, but at the same time, I can't see anything in the description of 28045 that would rule out using it. Can someone clarify the difference between the 2 procedure codes for me? Thanks.
 
From my understanding you use the excision, tumor codes from the musculoskeletal section when the excision goes down into or below the subcutanous tissue. As 28045 actually states "subfacial" I would only use this code if the documentation states that the keloid was excisized from below the subcutanious tissue or into the muscle. (I do not believe that what is being excized has to be malignant for it to be applicable.)
