Anyone know if there is documentation that states HCPCS S2068 requires an assistant surgeon? I have coded this procedure for quite a few case for different insurance cases and they have always paid for an assistant. United Healthcare says the procedure does not require an assistant. Any help on documentation would be greatly appreciated.
 
This was not a medicare case. We mainly bill commercial Ins. Most commercial insurance companies pay this without a problem with the exception of UHC. I am trying to find documentation on S2068. I know that medicare does not pay this code.
