Both RV and LV leads explanted. (only leads ever implanted).
How would this be billed (not dual chamber) and nothing re implanted.

The removal of only the RV and LV lead without replacement is coded with procedure 33235.

Procedure codes 33234 and 33235 are based on the generator type. You noted above this is a CRT-P device. This is considered a multi-lead device by coding definition. For example: if only the LV lead was removed and no other leads; it would still be coded to 33235. This is because you code these removal lead codes based on the generator type, not the number of leads removed.

Lead removed from a single lead generator: 33234.

Lead(s) removed from a dual or multi-lead generator: 33235
(regardless of the actual number of leads removed because the CPT instructions state to code based on generator type for 33234 and 33235).
