Hello, I need to clarify some coding points :
1. CPT 31622 bronchoscopy dx cannot be coded with another procedure since this CPT code says "Sep. Proc." and it's bundles with any endotracheal intubation CPT code at same surgery session. Many of our dr. perform bronchoscopy dx for the Vats procedure or repair of open hernial.
2. They also perform CPT 43235 EGD before hernial repair. Does EGD bundles with CPT 43334 repair paraesophageal hernial via thoracotomy ???
I was told CPT 43235 not allowed for 'scout endoscopy" per NCCI policy.
3. Last question where can I find the verbiage that is needed in documentation for Ultrasound guidance. Our dr. says Ultrasound guidance perform.... its not sufficient to code.
