Wiki Revision ulnar nerve transposition with placement of NeuraGen nerve wrap x2 code help

Need help on how to code for the nerve wrap. Would surgery be coded as 64718 and unlisted 64999 for nerve wrap? First page of op attached.

64910? That's what the Neuragen leads to. I couldn't get much out of the scanned dictation as it only has one page.

64718 does bundle into 64910

One more question? The provider used two nerve wraps. One at the proximal end of the ulnar nerve and the other at the distal end of the ulnar nerve . 64910 says each nerve. Would I bill bill 64910 x 1 with a 22 mod?
