Are there any offices performing this in the office setting and being reimbursed by Medicare?
NGS our "new" contractor cannot provide documentation supporting the allowed places of service for this code; they do however provide an allowed amount in the fee schedule listed under non-facility.
Not sure where to turn; can anybody point to a resource that lists CPT codes with the allowed place of service?
