How would you code removal of total knee and replaced with a hinged knee? do I code both 27445 and 27488?
please advise-thanks!
 
It appears that your physician did a Revision Arthroplasty of the knee for reasons other than infection, and what he put in was a Hinged Prosthesis. Code 27445 is for a Primary Knee Arthroplasty with a Hinged Prosthesis, not for a revision procedure. Code 27488 applies only to the removal of all the prosthetic components and cement, if any, with or without putting in a antibiotic cement spacer, done for an infected prosthetic joint with intentions of putting another knee prosthesis in at a later date, if and when the infection is felt to be "cured." Code 27487 is for a Revision Knee Arthroplasty for whatever reason, usually wear or loosening, though not usually done for infection, and the type of prosthesis implanted is not relevant. So 27487 would be the correct code.
