Hello I'm seeking guidance/ advice regarding the seventh character on a fracture and complication . Patient had a femur ORIF and a month later comes back for post-op and on xray there is a re-demonstration of the subtrochanteric femur fracture and interval displacement and screw cutout , the derotational screw has backed out.

Now would the 7th character be considered A because of the redemenstration of the fracture or G delayed healing K for non union and what the complication be considered a sequela or active?

T84.194_ other mechanical complication of internal fixation device of right femur
S72.21X_ displaced subtrochanteric fracture of right femur

99024 post op

Any thoughts????

You would have to code to what the provider actually states it is. Did they call it a complication, nonunion, malunion, delayed healing, etc.? Did they have a slip/trip/fall and cause a new fracture or displacement of the screw? Do they have osteoporosis?
