I have a chart where the patient comes to the ED, uses heroin and just found out she is pregnant. She wants a referral so she can quit using. She also is having nausea and vomiting. Can I report Hyperemesis gravidarum, even though the MD doesn't document "Hyperemesis gravidarum"?
 
If the Dr doesn't state Hyperemesis gravidarum than NO you can't code that. You can used the unspecified vomiting in pregnancy 643.93 if it just states she is having vomiting during pregnancy and the note is very unspecific.
