Did code Q4098 change? I saw it had been deleted but I can't find a new code. I was reading over some of the CMS information and it mentions this code not being added to the CAP, I'm not even sure what CAP means. Is Medicare no longer paying for the iron dextran 50mg? Should I be using Q4082 Somebody please set me straight! Thanks
