We administer Neulasta prophylactically to patients receiving chemo meds with high risk of inducing neutropenia. We have always used V07.8 as the primary dx followed by the cancer dx code. I am having trouble finding any written guidelines to follow for ICD-10 regarding coding of prophylactic administration of Neulasta (Pegfilgrastim). I do not like the translation of
V07.8 to ICD-10 - it's not a good fit.
Does anyone have a suggestion or a resource?

Prophylactic Neulasta injection for infection prevention

Coding Clinic, Second Quarter ICD-10 2014 Page: 10 Effective with discharges: May 26, 2014


Question:

A patient who is receiving ongoing chemotherapy for lung cancer presents for Neulasta injection to prevent infection. The patient receives the Neulasta via a subcutaneous injection. The advice provided in the Second Quarter 2010 issue of Coding Clinic for ICD-9-CM advised coders to report code V07.8, Other specified prophylactic or treatment measure, for an encounter for Neulasta administration to prevent an infection. However, there does not seem to be an equivalent code in ICD-10-CM. What are the appropriate diagnosis and procedures codes for an encounter for Neulasta to prevent infection?



Answer:

Assign code Z76.89, Persons encountering health services in other specified circumstances, as the reason for the encounter. Assign also the appropriate code assignment to describe the patient?s cancer. In this case, assign code C34.90, Malignant neoplasm of unspecified part of unspecified bronchus or lung.



If desired, assign ICD-10-PCS procedure code as follows for the subcutaneous injection of Neulasta:


3E013GC Introduction of other therapeutic substance into subcutaneous tissue, percutaneous approach

Define "long term"...

Regarding the suggestion to use Z79.899 or for the section Z79.8 (other long term (current) drug therapy)...what is considered long term? Can it be used when the patient has had, for example, only two Neulasta treatments, because we know this will be long term? Or do we have to wait a specific time frame before using one of these "long term" dx? Thanks.
