Wiki Procedure Code for Nerve Bock, Left Trapezius

These branches of nerves are confusing me.
This is the procedure note I have to go by "patient received a left sided nerve block and trigger point injection in the left trapezius." Is this enough information to code by, and if so what CPT code am I looking at?
I first had suprascapular nerve injection, but I wasn't sure that was correct based on the nerve branches or documentation. This is not something our ER physicians do often, so I am just not familiar with this. Any help is greatly appreciated, thanks

Yes. And assuming the doctor can remember what they did, they also need to amend that note.
