On a Preventive exam such as 99393, could 2 modifiers be applied: (such as 25 and EP)? I've been getting denials from MissouriCare medicaid, even though both of these modifiers are relevant. What should I do here? Maybe add a modifier 99 or just remove the EP? This keeps happening with claims where vaccinations or other preventive services were done. What should I do?
