Pt in for routine pregnancy check in second trimester. Provider states in A/P: "History of acute lymphoid leukemia in remission." There is no treatment aimed at that this visit or any other mention in note, just in A/P. Do I code this with O code or Z (personal history) code? I've researched a bit and I think I use O.

O9A.112, Malignant neoplasm complicating pregnancy, second trimester
C91.01, Acute lymphoblastic leukemia in remission

I really need confirmation of this coding. I'm feeling a bit uneasy about using the O code, but again, from what I've researched and through reading ICD-10 guidelines, I believe the above coding is correct. Do you agree?

Thank you so much for your input!

The dx codes selected are correct. The provider would need to document that the condition is not complicating the pregnancy for there to be any other choice.
