United Healthcare keeps denying our claims with primary ICD 10 codes of Z3490. Does anyone have a list of pregnancy codes they accept?

They're going to deny that code because it's unspecified as to whether it's the first or subsequent pregnancy and unspecified trimester. Insurance companies are going away from accepting these unspecified codes. Z34.01, Z34.02, Z34.03, Z34.81, Z34.82, or Z34.83 should be used instead based on if it's the first pregnancy or a subsequent pregnancy and the trimester. You should also include the weeks of gestation code Z3A.xx with any OB related charges. If there are any tests performed on the same day, they also want a code from category Z36. I usually use Z36.89 for UAs and most labs. Take a look at those sets of codes. There are some Z36 codes that are specific to certain labs and ultrasounds, such as Z36.85 for group B strep and Z36.87 for a dating ultrasound. Hope this helps!
