Hi there
I am looking for feedback on where your surgeons document the post op pain management reassignment to anesthesiology when your anesthesiologists perform the post op Pain blocks and rounding for management of the catheters? This documentation is needed per NCCI Chapter 2 page 5? Do they document this in the operative report or orders? If in orders are your surgeons submitting the orders or is a resident?
We are looking at changing our practice and wanted to know what the standard usually is.

The request for a block should be in the op report. Post-op pain management that takes place the following day should be documented by the anesthesiologist (01996).
 
I have a similar question. The surgeon performs a circumcision on a 10 month old under general anesthetic. The anesthesiologist performs a nerve block (while still under general anesthesia) for post-op pain control. How would you code and bill the nerve block - under a procedure revenue code or an anesthesia revenue code?
