When reporting medication reconciliation for transition in care calls some payers offer incentives for the med rec with cat II code 1111F ,however the caveat is that it can't be reported with the TOC codes 99495 or 99496 .Our thoughts are then to have the TOC nurse bill Med rec with the initial telephone call (within 2 business days )
Would this change the POS for a telephone call ?
Appreciate any help/direction
