I had quetion related to PORS messure 436.i got combo report of ct chest and ct abdomen of same dos same time of exam and same physician.should i report pqrs 436 twise or one time

Did anyone knows about this PQRS 436 reporting.how it report if two procedure done on same day by same physician and has generate in single report.Please help me out.
