Wiki PNCL Stone Size

I am trying to decide on CPT code 50080/50081 for the size of stone. I was told if it is commercial insurance you can add the stone size up and if it Medicare you have to use the largest stone size. Is that how you are all doing this? Any advice would be greatly appreciated.
