If a physician explants the PM generator and RA, RV and LV leads would I report 33235, 33234, 33233? Also, if physician implanted new RV lead and connected to the explanted CRT-P generator and anchored to left upper chest externally?

Correction, should I report codes 33235, 33234 and 33216 since insertion of transvenous electrode includes connection to the generator. (not stipulated if subcutaneous insertion of generator)
