Our dr did a 99396, 99213 and 93000, 69210 and 82274. how do i code this?

88274, qw
93000
69210, 51
when i do coding check, it states that 99396 must have a modifier, but then so does the 99213 and then 93000 is a component of 69210 and needs modifier... i tell him not to do this in one visit, but he does not listen. Please help me
AS IT IS CODED NOW, the system is stating it is WRONG

Hi Danielle,

Would you post the visit note? That way we can see if there is a legitimate 99213 to go along with the prevent visit. Also, was the patient scheduled for a preventive visit? Or was he/she scheduled for another reason?

If your documentation does support all of these services then it should be billed
