In ICD10- what dx code would you use when the pt is PCOS but is starting infertility treatment needing cycle management of her ovaries with follicular u/s's and labs using Clomid and possibly heading towards have an AI? ICD10 has an excludes 1 note under the heading of infertility for the PCOS. Should I use only the unspecified infertility code of N97.9 or use only the PCOS code alone?

Polycystic ovary disease (PCOS) is a hormonal imbalance in women that is thought to be one of the leading causes of female infertility. Polycystic ovary syndrome causes more than 75% of cases of anovulatory infertility.

PCOS usually causes infertility associated with anovulation, and therefore, the presence of ovulation indicates absence of infertility, though it does not rule out infertility by other causes.

So I would suggest PCOS is the appropriate Dx for this visit.
