I need help with a patient that came into the dr office and received a hyalgan shot. The patient is in a nursing home. Medicare paid the whole claim and then only recouped on the Hyalgan code J7321. resubmitted the claim for that line item using a modifier 26 and it was still denied. Can someone please help me or steer me in the right direction to find out what modifier to use . Any help will be much appreciated.
