BCBS is denying cpt 58661 stating "Contractual Obligation - The benefit for this service is included in the payment/allowance for another service/procedure that has already been adjudicated

Any suggestions

Couple of things here...
58180 is OPEN supracervical hysterectomy with or without removal of tubes and/or ovaries
58661 is LAPAROSCOPIC removal of adnexal structures (tubes and/or ovaries)

1) You clearly cannot bill for the same service twice, which you would be by billing both codes
2) Even if 58180 did not include removal of tubes and/or ovaries, it would be highly suspicious to have an open and a laparoscopic procedure during the same surgery. If the physician attempts laparoscopic, and needs to convert to open, then you only bill the open.
