We have a Medicare patient that has a port and we are starting them on Orencia. I spoke with our RN, and she tells me that she is doing an IV push of Heparin J1642, 96375 through the port. Should I bill for the IV push 96375 and the port flush 96523 or would the IV push be considered part of the port flush?

A port flush is incidental to the infusion. You can bill for the supply of the heparin itself, but you can't charge for the port flush. You also cannot code it as a push because that's not what it is - a push is a therapeutic administration of a drug, not one given to clear the line or keep the vein open.
