The patient was scheduled as a lap adjustable gastric band placement. However, due to massive adhesions, surgeon needed to convert to open procedure. There is no cpt code for OPEN placement of adjustable gastic band. Should I use 43770 and use dx code V64.41 or must I resort to unlisted procedure code.
 
How about 43843? There's a note at the end of the Bariatric Surgery section that says...

"For open gastric restrictive procedure, without gastic bypass, for morbid obesity, other than vertical-banded gastroplasty, use 43843."

If that doesn't work, try using 43770-22 to show that the surgeon had to open the patient up.
