Stress report signed off by Dr North on 5/12/15. Dr West did nuclear interp, which included reviewing/interp of stress portion. He signed off on 5/14/15. (all done in their office: 26/TC)

We are billing this out: 78452,93015,A9500x2 under Dr West on 5/12/15.

We do not use adenosine. My docs haven't for years.If we did a pharmalogical stress we would use lexiscan J2785 or dobutamine J1250 dependng on the patient.
