I am trying to find justification on fee schedule for BR code 76496 modifer 59. Has anyone successfully gotten this code paid by No Fault, and if so what written justification have you given them. My client has charged them $700.00 and I need to be able to show that is appropriate and I have not been able to find anything on this. Any help you can give me on this would be greatly appreciated. Thank you!
