Hello! I need help with a Diagnosis code. Pt (mother) gave birth and her baby passed away due to health complications a week after birth. Pt (Mother) is now in office for a post-partum visit, is there a diagnosis to report for the maternal record of the baby's death? Would I use Z39.2 (Postpartum Follow up) and Z63.4 (Disappearance and death of family member)? Your advice on this is greatly appreciated! Thank you!

Did the provider list reason why newborn died? check out blocks of P84-P95. But from info above you gave I d code it P92.6 failed to thrive after birth. Check out dx Z84.82 for the babies death of SIDS. Ahh coding for the baby here. There is a dx code for postpartum depression for mother illness 090.6 or F53.0

Hi Lady T, thank you! My thought is that you are not allowed to use P codes on the maternal records…P codes would only apply to the child’s record, is that correct?

Hello, I am not an expert, just thinking. I would code it as postpartum visit O99.89 with R4589 for Other signs and symptoms of emotional state if Md documented pt’s sorrow etc. Sorry, I don’t have ICD 10 book with me. And Z63.4
