We're having issues with the new lcd trying to figure out how to code the primary dx. We have a patient with group 3 dx PVD I73.89 so we added the Q mod and are billing 11719 for trimming of nondystrophic nail but have to have a group 1 dx primary. Do we just use L60.8???
