I am billing nerve tests for a DO. Novitas/Medicare is rejecting all claims for invalid or missing information. They are lumping 95903/95904 with 95861 stating that they can not be performed on the same date.

As of January 1st, 95903/95904 are inactive. You will want to refer to the new codes 95907-95913. Also, as of January 1, 2012, 95860-95864 are only to be billed when an EMG is done with out NCV.
